The GP Out of Hours Service covers 23 practices in the Eastern Cheshire Area. The out of hours is for urgent medical needs that cannot safely wait for the patients own GP surgery to open. The out of hours operates Mon- Thurs, 18.30-08.00, Fri 18.30 – Mon 08.00 including Bank Holidays.
The team consists of Call Handlers, Drivers, Triage Nurses, Nurse Practitioners and GP's, providing excellent care to the patients within Eastern Cheshire either by telephone triage, base appointment or home visits. Last year the team had contact with over 23,000 patients.
Patients that arrive at A&E and streamed to the service is currently 8% of the total A&E attendances, this percentage continue to rise to support the A&E department and patients, ensuring patients are in the correct place for the care required.
In December 2013 the service launched the Acute Visiting Service (AVS). This service was lunched to support the North West Ambulance Service (NWAS) in providing the best care for patients that did not need to come into hospital but require medical attention. The patients are seen at their own place of residents and cared for by our team of GP’s . Since 2013 the service has safely identified 5625 patients who needs can be met at their own place of residence. Response from the service has been very positive both from clinical staff and patients.
We are very proud of all the services we provide; our team work ethic is very strong, with competent, experienced and friendly staff. Our staff retention is an example of the team success and supporting each other is our greatest strength. Our clinical staff meets on a monthly basis to discuss clinical cases and learning. Patient questionnaires are sent out to patients providing valuable feedback for appraisals and revalidation.
